Clinical Education Contracts

The following material, provides information and instructions to students and their supervisors for the administration and processing of the clinical education contracts for their clinical experience in the master’s program. 

As you know, you will complete 5 clinical practicums during the three-year program: three pediatric and 2 adult.  It is the student’s responsibility to secure clinical education contracts between the clinical sites and CSUN.  (Please note:  this document is in addition to the “Supervisor Nomination Form” that the student and supervisor are responsible for completing, prior to the start of each practicum.  The “Nomination” forms for pediatric and adult practicums are available for downloading from the Advisement Site on Blackboard.)

The clinical education contracts should be downloaded and taken to the supervisor at your clinical site for completion by the appropriate personnel.  Before downloading the appropriate contract, be sure to check with your supervisor to determine whom is responsible for executing a legal contract at your site. This information will determine whether to use the “general” clinical contract, or the “therapist” contract. 

General Clinical Education Contract

This contract should be used, if the school’s administration (HR, superintendant, etc.).or its legal representative is required to sign. 

Therapist Clinical Education Contract

This contract should be used, when it is appropriate for your supervisor (SLP) to sign. 

(Please note:  it is the student’s responsibility to make sure that the correct contract is signed, or CSUN’s contract office will return it to the site, unsigned.) 

Once the contract is signed by the appropriate personnel at the clinical site, the original should be sent to the address on the last page (signature page) of the document.  The address is the Contract and Purchasing office at CSUN. 
Upon receipt, the contract office will review the document, sign it and return a fully-executed copy to the site for their files. They will also send the Communication Disorders & Sciences department a copy for our files.
